Sergeant David W. Shaw, while on a working confined space rescue, climbed down the manhole ladder into the underground utilities vault & collapsed. Tragically, Sergeant Shaw died of CO2 asphyxiation.Sergeant David Winston Shaw was 30 years old . Number of years served are unknown at this time.
